Welcome to John Paul II. The Athletic Department at John Paul II offers a variety of interscholastic Athletic Programs. For your son or daughter to participate they will be required to obtain a physical evaluation form. This form must be signed by a health care provider. These are consent and release forms. These forms must be turned into the schools office before your child can participate in any interscholastic activity. These forms are available to you below.

We hope your child chooses to participate in a sport. It can be a great experience.
